This is the large (12-inch) aluminum "Prophet 5" name badge, which goes on the back panel of Rev 1 and 2 keyboards; this is not the correct badge for Rev 3 Prophets. With self-adhesive backing, and front protective film.

NOTE: This badge is NOS (new old stock), and the self-adhesive backing may have lost its adhesion. If it is not 'sticky' enough, simply peel off the backing and then apply double-sided Scotch tape to mount the badge.
