Name badge, for Sequential Circuits keyboard. This is the small aluminum badge, which goes on the front panel of a Prophet 5, Prophet 10, or Pro One keyboard. With adhesive backing, and front protective film.

NOTE: This badge is NOS (new old stock), and the self-adhesive backing may have lost its adhesion. If it is not 'sticky' enough, simply peel off the backing and then apply double-sided Scotch tape to mount the badge.
